A CA-16 is a form given to you by your employer for authorization for examination and/or treatment by a qualified medical facility/provider. It only covers services given by the facility on the form. The form must be taken with you to the facility. They will complete the back pages and you will be required to return this to your supervisor. You receive immediate care, and it is covered by your claim even though you do not have a case number yet. Any care by another facility will need a new CA16 (issued by occupational medicine) or to be approved by your claim.
